Investigations

The Authority may undertake or be required by the Minister to undertake an investigation into the safety, quality and standard of healthcare services, if there is a serious risk to the health and welfare of service users.

We will make recommendations based on the findings of our investigation.  We always publish the findings of our investigations on our website.

Investigations currently being carried out:

• Investigation into the quality and safety of services and supporting arrangements provided to patients requiring acute admission and receiving care in the Emergency Department of the Adelaide and Meath Hospital, incorporating the national Children’s Hospital (AMNCH)

Investigations carried out to date: 

• Report of the investigation into the quality and safety of services and supporting arrangements provided by the Health Service Executive at Mallow General Hospital
• Report of the investigation into the quality and safety of services and supporting arrangements provided by the Health Service Executive at the Mid-Western Regional Hospital Ennis
• Investigation Report into the Pathology Service and the Symptomatic Breast Disease Service at University Hospital Galway
• Investigation Report into the care received by Rebecca O’Malley, Symptomatic Breast Disease Services at the Mid Western Regional Hospital Limerick and the Pathology Services at Cork University Hospital
