Residents’ questionnaires are sent in advance of announced inspections to allow residents to provide feedback on what it is like to live in a designated centre.

Residents, relatives, family, advocates or carers can also complete the form at any time outside of the inspection process and send it to us. You can email or post it to us, or alternatively, you can give the completed questionnaire to an inspector on the day of an inspection.

To submit your survey electronically, download and open it. When you have completed it, save it, then click the ‘submit’ button on the back page of the downloaded file to automatically attach it to pre-prepared email to us. Or you can attach your completed survey in an email to us. Alternatively, you can print it, complete it by hand and post it to us.
